Forms & Letters (non-covered administrative service)
Some requests—like employer return-to-work forms, school/work letters, accommodation notes, and similar paperwork—are administrative services performed outside of session. These are not billed to insurance and are billed directly to you at $100/hour in 15-minute increments ($25 minimum).
• Most simple forms/letters take about 10–15 minutes → $25.
• If a request looks like it may exceed 15 minutes, I’ll let you know first so you can approve it.
• Payment is charged to your card on file when the form/letter is completed.
• Requests for copies of medical records are handled separately under HIPAA’s right-of-access rules and are not part of this admin fee.
Insurance & Billing:
For clients using insurance, billing may be handled through Alma, Headway, or a direct NuWave Counseling LLC payer contract, depending on your plan, state, and product line. Insurance availability varies by payer and eligibility. Copays, deductibles, and other out-of-pocket costs are based on your specific plan. Good Faith Estimate
You have the right to receive a “Good Faith Estimate” explaining how much your medical care will cost Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services.
-
You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees.
-
Make sure your health care provider gives you a Good Faith Estimate in writing at least 1 business day before your medical service or item. You can also ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service.
-
If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.
-
Make sure to save a copy or picture of your Good Faith Estimate.
